Our school is administering the Community-Level Youth Development Survey (CLYDE) on April 1-April 4, 2025. Through the anonymous survey, students will be asked about their attitudes, behaviors, and experiences in their community. The results of this survey will help plan programs and activities for youth. 

Students will be asked to fill out a survey on a computer during regular class time. The survey takes one class period or less to complete. 

Completing the survey will cause little or no risk to your child. The only potential risk is that some students might find some of the questions to be sensitive. 

Your child’s privacy is protected. Students will not put their names on the survey, and no class or student will ever be mentioned by name in the report of the results. 

The survey is voluntary. There are no negative consequences for you, or your child, if your child does not take part. Students can skip any question or stop taking the survey at any point.

If you do not want your child to take part in the survey, fill out and return the form below to the school.  Signing and returning this form will dismiss your child from taking the survey, and they will instead complete a quiet academic activity during survey administration. If you have no objection to your child taking part in the survey, you should do nothing with this form.
